Conditional Job Offer
This offer of employment depends on the following:
1. Completion of an acceptable pre-employment physical and negative TB Test
2. You must get your physical and TB test where Central State Community Services sends you. Central
State will pay for both the physical and TB test. (Depending on the results of the physical, you may
have to apply to become certified as vocationally rehabilitated (Chapter 9 of Michigan Worker’s Comp.
Act). If this screening is necessary, Central State will pay for it.
3. Verification or completion of:
a. References
b. Competency test (Reading and Writing)
c. Past Employment
d. Licenses and Certificates
e. Background Check (Requires fingerprinting)
f. Current, valid, and unrestricted driver’s license (you must also be insurable by the insurance
company used by Central State Community Services.
g. Good moral character and suitability to this position
h. Identity and right to work in the U.S.
You must fully cooperate with all these requirements. If you do not cooperate fully, within 10 days, this job
offer will automatically be withdrawn. Central State Community Services must be able to verify the accuracy
and truth of all information provided throughout the hiring process. Information obtained will be kept
confidential and only released as required by law or by contracting agency.
You will be provided a schedule for required trainings and must attend and pass them all. If you fail to attend
and pass these trainings, including Recipient Rights, CPR, First and Blood Borne Pathogens training which
must be passed within 30 days of hire, you will be removed from the schedule and your employment may be
terminated. There shall be no discrimination against a recipient or an applicant for services based on gender,
pregnancy, race, color, religion, national origin, citizenship, mental or physical disability, military service,
veteran status, political affiliation, familial and marital status, age, sexual orientation, gender expression, height,
weight, or genetic information.
This offer does not change anything about the at-will status of employment.
A criminal history does not necessarily mean you cannot work for Central State Community Services.
